This FORUM focuses on short term in-patient skilled nursing facility rehabilitation. Some individuals are unable to return directly home following an elective surgery or unexpected hospitalization, as they require the daily services of skilled nurses and therapists to begin their recovery. A vast majority of these patients return to a community setting following short term in-patient rehabilitation.....

Are you planning to have a hip or knee replacement? Have you experienced an unexpected hospitalization? Short term in-patient skilled nursing facility rehabilitation may be just what you need. There are times when an unexpected injury, serious medical condition, prolonged hospitalization or an elective surgery such as a total hip replacement leaves you or a loved one in need of rehabilitation to regain the health, strength and skills to lead an active and satisfying life. The expert nurses and therapists of BayView Rehab at the Samantha R. Wilson Care Center are here to help.

BayView Rehab at the Samantha R. Wilson Care Center ~

What is a Skilled Nursing Facility?

Skilled nursing facilities or SNF's are health care facilities that are licensed,

Multidisciplinary Team Approach~

A serious medical condition or trauma can leave you having to relearn your basic skills of daily living. At BayView Rehab, we understand how stressful and challenging a sudden illness or life changing medical event can be for both you and your family. A multidisciplinary team comprised of physicians, nurses, nursing assistants, physical therapists, occupational therapists, speech language pathologists, dieticians and social workers tailor a rehabilitative plan specific to your needs. Our team of experts focuses on assisting you to regain lost skills, and work towards restoring your strength, mobility and function. We strongly believe that you are an important member of the team, and should be involved in setting goals and working in harmony with us towards your recovery. Our team of highly specialized professionals has all disciplines working together to offer you the best care and outcome.

Post Surgical Rehabilitation Unit ~

BayView Rehab offers a rehabilitation unit for patients who have elected to have knee, spine, shoulder or hip replacement surgery. All therapy is administered by highly qualified specialists with advanced training in orthopaedic rehabilitation. We are dedicated to helping orthopaedic patients build their strength, coordination and mobility to get them back home, to their family and to an active and rewarding lifestyle. This newly renovated area is centrally located near the rehabilitation gym, the courtyard and is easily accessible for family and friends to visit.

Physical Therapy~

Physical therapy (PT) focuses on regaining mobility and strength following surgery or a prolonged hospitalization. Physical therapy provides for the expert examination and treatment of musculoskeletal and neuromuscular problems that affect your ability to function as well as you want in your daily life. Treatment modalities may include gait and balance training, neuromuscular re-education, prosthetic or orthotic training and other modalities. The goal of physical therapy is to achieve greater functional independence. Your physical therapist will recommend assistive equipment if appropriate and will provide training and instruction for these devices. Physical therapy is very effective in the treatment of physical dysfunction or injuries such as muscle, tendon, and ligament tears; post-operative surgeries such as joint replacements; arthritis; fractures; Benign Paroxysmal Positional Vertigo or other vestibular disorders; traumatic brain injury; spinal cord injury or amputation.

Occupational Therapy~

Occupational therapy (OT) focuses on helping individuals achieve the "skills for the job of living" at home, at work or pursuing leisure activities. The occupational therapist's goal is to increase your independence in meaningful activities ranging from bathing, dressing, to leisure and community pursuits. The therapists analyze your physical, environmental, psychosocial and cognitive factors to identify barriers to everyday activities. Occupational therapists can offer a comprehensive evaluation of your home, make recommendations and provide training for adaptive equipment, and education for family members and caregivers. Our therapists believe it is important to focus on adapting the environment to fit you

Speech Language Pathology~

Speech Language Pathologists (SLP), also known as speech therapists, evaluate and treat disorders related to communication or speech, swallowing (dysphasia) and cognitive aspects of communication (expressive aphasia). Speech therapy usually involves individual therapy, training and education, to overcome or learn new compensatory strategies. Speech therapy is successful in treating a wide variety of disorders resulting from stroke, traumatic brain injury or other neurological conditions including Parkinson's disease, multiple sclerosis and ALS.

Comprehensive Nursing Services

We have a team of specially trained nurses who provide around-the-clock nursing care. They are usually the first of the team members that you meet upon arrival and the last you see at the time of your discharge home. Comprehensive nursing services are often necessary when recovering from acute injuries or complex medical conditions. IV therapy, wound care, pain management and edema management are example of the skilled services provided by our qualified nurses.

Will my insurance cover in-patient rehab?

It is important that you verify with your health care provider what benefits you have for short term rehabilitation at a SNF. If Medicare is your primary health care insurance there are number of criteria requirements that must be met in order for you to qualify for a SNF stay. Some examples include:

• You must have Medicare Part "A" and have days left in your benefit period for use.

• You have had a qualifying in-patient hospital admission of 3 consecutive nights.

• Your doctor has ordered the services you need for SNF care requiring the skills of professional personnel such as registered nurses, physical therapists, occupational therapists or speech-language pathologists.

• You need these skilled services for a medical condition that was treated during your qualifying

3-night hospital stay or started while you were getting SNF care for a medical condition that was

treated during a qualifying 3-night hospital stay.

• The skilled services must be reasonable and necessary for the diagnosis or treatment of your

• You get these skilled services in a SNF that is certified by Medicare.

Many private insurance plans follow similar criteria to Medicare for a short term in-patient stay at a skilled nursing facility. It is important that you contact your benefits administrator to verify what health care is covered, your personal financial responsibility and who is a provider within your network.
